Case Manager / Coordinated Family Support (CFS) Specialist

Location: San Diego County
Position: Full time, 40 hours per week
Pay: $62,400
About the Position:
My Brother’s Keeper Transitional Housing is seeking a compassionate, organized, and dependable Case Manager / CFS Specialist. This combined position provides case management to young adults transitioning from foster care into independent living and Coordinated Family Support services to adults with developmental disabilities and their families.
The ideal candidate builds strong professional relationships, follows through on participant needs, and maintains accurate documentation. This position requires in-person services and travel throughout San Diego County.
Responsibilities:
Transitional Housing Case Management
- Help participants develop and pursue goals related to education, employment, budgeting, health, and independent living.
- Complete assessments, individualized service plans, NSP's, TILP and Individual Relocation Plans (IRPs).
- Conduct regular participant visits and follow up on progress, concerns, and service needs.
- Connect participants with community resources, benefits, healthcare, and other supports.
- Coordinate with social workers, attorneys, service providers, and program leadership.
- Support stabilization planning, conflict resolution, and successful transitions from the program.
- Maintain timely case notes, ETO entries, participant files, and required reports.
- Communicate housing concerns to the Housing Specialist and coordinate participant follow-up.
Coordinated Family Support
- Assist adults with developmental disabilities and their families in identifying needs and accessing services.
- Provide support consistent with authorized services and individual goals.
- Help coordinate appointments, referrals, transportation resources, and community supports.
- Support daily living skills, self-advocacy, and greater independence.
- Help families develop routines, identify backup supports, and plan for changes in caregiving.
- Communicate with Regional Center service coordinators and other providers, with appropriate consent.
- Complete accurate service notes, progress updates, time records, and billing documentation.
Qualifications:
- Experience in case management, foster youth services, developmental disability services, family support, or a related field preferred.
- Bachelor’s degree in social work, psychology, human services, or a related field preferred.
- Must meet applicable program education and experience requirements.
- Strong communication, organization, problem-solving, and documentation skills.
- Ability to work independently and collaborate with participants, families, and service providers.
- Ability to maintain confidentiality, professional boundaries, and respectful relationships.
- Comfortable using electronic documentation systems and basic office software.
- Valid driver’s license, current automobile insurance, and reliable transportation.
- Ability to meet applicable background clearance and employment requirements.
- Bilingual English/Spanish skills are a plus.

Schedule and Work Setting
This is a full-time position, primarily during normal business hours. Work takes place in the office, participant residences, family homes, and community settings. Regular travel throughout San Diego County is required. Occasional schedule adjustments may be needed to meet participant and family needs.
