Description
The Certified Community Behavioral Health Clinic (CCBHC) model alleviates decades-old challenges that have led to a crisis in providing access to mental health and addiction care. As an integrated and sustainably-financed model for care delivery, CCBHCs:
- Ensure access to integrated, evidence-based addiction and mental health services, including 24/7 crisis response and medication-assisted treatment (MAT) for addiction.
- Meet stringent criteria regarding timeliness of access, quality reporting, staffing and coordination with social services, criminal justice and education systems.
- Receive funding to support the real costs of expanding services to fully meet the need for care in their communities.
RESPONSIBILITIES:
- Engage in outreach and engagement to veterans and their families.
- Interact with Veterans, family members, significant others, and VA and non-VA community health care providers, community agencies and resource programs.
- Actively participate as a member of the CCBHC interdisciplinary team.
- Provide evidence-based trauma assessment and treatment.
- Serve as a liaison between Veterans and/or their families and VA and community resources in order to ensure thorough delivery of services.
- Provide consultation as needed to other treatment team and staff members regarding psychosocial needs of veterans and/or their families.
- Assist administrative staff in collection of information for program effectiveness.
- Maintain accurate clinical records in compliance with all city, state, and federal regulations.
- Participate in all training programs for the Whole Family Health Model.
- Attends individual supervision with clinical supervisor.
SKILLS AND KNOWLEDGE:
- Highly skilled and knowledgeable in treating veterans with mental illness, substance use and co-occurring disorders with a strength based, whole health approach.
- Crisis intervention response, support and follow up.
- Ability to effectively communicate and interact with clients, colleagues, public, other agencies and their staff, and third-party insurance personnel.
- Cultural competence with Latino and other cultures.
QUALIFICATIONS:
Master’s degree in social work, counseling, education or other relevant human service profession, with two years’ experience in veteran services. Ability and willingness to do home visits. Ability to work independently, as well as contribute to a team. Very good interpersonal/communication and strong clinical skills.
Veteran strongly encouraged, bilingual Spanish is highly preferred.
HOURS PER WEEK: This is a full-time 40-hour week exempt salary position, utilizing a flex schedule.
TRAVEL REQUIREMENT: Moderate degree of travel to community and family homes
