Description
The Certified Community Behavioral Health Clinic (CCBHC) model alleviates decades-old challenges that have led to a crisis in providing access to mental health and addiction care. As an integrated and sustainably-financed model for care delivery, CCBHCs:
- Ensure access to integrated, evidence-based addiction and mental health services, including 24/7 crisis response and medication-assisted treatment (MAT) for addiction.
- Meet stringent criteria regarding timeliness of access, quality reporting, staffing and coordination with social services, criminal justice and education systems.
- Receive funding to support the real costs of expanding services to fully meet the need for care in their communities.
RESPONSIBILITIES:
- Conduct community outreach activities aimed at target population through the distribution of materials and information via presentations at local organizations including natural gathering sites, places of worship, and other community-based programs.
- Identify new community partners and market upcoming events and community-based activities.
- Support client satisfaction surveys to identify program strengths and areas to improve. Provide feedback to multidisciplinary team based on client observations and interactions.
- Assist program participants with accessing needed community services (housing, medical, vocational, food, clothing, etc.) through network development, referral and follow up.
- Accompany participants to appointments, as appropriate.
- Actively participate as integral part of the CCBHC multidisciplinary team including family care planning and collaboration.
- Perform basic accountability tasks, include compiling and maintaining all necessary records and assist administrative staff in collection of information for program effectiveness.
- Participate in all trainings for the Whole Family Health Model.

- Prepare accurate and timely documentation, reports and other written material as assigned.
- Work independently and conform to all applicable safety and accountability measures.
QUALIFICATIONS:
2 years’ experience in outreach and fieldwork in the behavioral health field. Minimum high school diploma or equivalent.
Bilingual Spanish strongly encouraged.
HOURS PER WEEK: This is a full-time 40-hour week utilizing a flex schedule.
TRAVEL REQUIREMENT: Moderate degree of travel in community
