Position Overview

YWCA’s Senior Services Program empowers seniors to maintain healthy and independent lives in the safety and comfort of their own homes. The Seniors Program Advocate facilitates seniors’ access to services across agencies and systems, leverages resources, and coordinates caregiving with medical providers, family members, and other providers. In this role, you will be on a mission to use your creative problem-solving skills to support the dignity of vulnerable seniors.

Key Responsibilities & Deliverables/Outcomes

Strength-Based Case Management
• Build consistent and trusted relationships with senior program participants and engage with appropriate family members to provide decision making and connections.
• Carry an active case load of 60 to 75 senior participants.
• Meet with clients regularly with home visits and by phone.
• Provide Options Counseling to assist with long term planning, including a needs assessment and action plan.
• Assess needs and eligibility for Oregon Project Independence, Medicaid, Meals on Wheels, Family Caregiver

Support, Energy Assistance, and other social programs.
• Monitor service plans for clients; includes but is not limited to periodic plan reviews and reassessments.
• Work with in-home care providers to maintain service plans and meet the needs of the client.
• Encourage and facilitate client participation in community activities and support groups.
• Communicate available resources to clients through phone calls, emails, and in-person appointments.
• Develop and maintain a comprehensive understanding of available social support services for older adults including Older Americans Act direct services.

Accurate and Timely Service Reporting
• Complete accurate, legible, and timely documentation of all services provided to every client in YWCA's case management information system as well as in county and state systems.
• Meet all client service standards as required by contract regarding quantity, type, and quality of services to be delivered to clients and enter case notes within the contractually obligated 3-days of services provision.
• Stay up to date on all required training, including YWCA’s 12 hours a year of social justice training.
• Adhere to all required confidentiality standards including HIPAA, Medicaid, and OPI, when handing client information.
• Ensure all productivity requirements are met.

Participatory Teamwork and Responsible Representation
• Engage consistently in anti-racism practices and approaches with internal and external work based on a deep understanding of historical and present-day structural racism.
• Attend and meaningfully participate in all scheduled meetings with department, agency, and external partner teams.
• Maintain all required training and certification plus 12 hours of YWCA Social Justice Training annually.
• Serve as program liaison to collaborating organizations including district centers, social service agency partners, government, and community groups.
• Uphold YWCA of Greater Portland’s mission, vision, and values by acting with integrity, accountability, and responsibility toward resident participants, service partners, fellow employees, volunteers, donors, and everyone the organization engages with.
• Perform other duties as assigned.

● Regulates personal emotions and practices self-care while enduring high amounts of stress and vicarious trauma.
● Brings an intersectional lens to the work that focuses on lived experience, resiliency, and opportunities for calling in and deeper learning.

Skills, Knowledge, and Experience
● Required: 40-hour Domestic Violence Advocate Training (provided).
● Required: Enhanced background check.
● Experience working in systems that serve the elderly or training and education in social work or other human services fields.
● Bilingual or bicultural skills in Spanish, Chinese, Korean, or Vietnamese preferred.
● Ability to effectively work within a strength‐based, trauma informed, integrated family support service delivery model.
● Demonstrated culturally responsive communication skills (both written and spoken)
● Computer skills include working knowledge of Microsoft Office Suite, Google Workspace, virtual meeting tools and the ability to learn multiple HMIS systems.
● Ability to work independently and be self-motivated.
● Ability to organize, prioritize and complete multiple and varied tasks.
● Desire to actively participate in self-reflection, continued education, and professional development.
● Ability to build rapport with people from diverse cultural and economic backgrounds; consistently treat others with respect and dignity.
● Ability and willingness to work as a team member and support the mission and goals of YWCA of Greater Portland.
● Flexibility, kindness, discretion, and a sense of humor are essential.

Job Conditions
● Hybrid position in Portland Metro Region that requires 2 to 3 days a week in the Multnomah East County Health Center office and frequent visits with program participants, colleagues, and other partners at varied community locations.
● Must be able to provide own transportation to visit multiple Multnomah County sites, travel is reimbursed.
● Laptop, phone, and other work-from-home equipment as needed will be supplied.
