Description

The RSC position is responsible for conducting individual and/or community assessments to determine service needs and ensure that service delivery addresses the current client/population and service needs.  RSC will deliver programs in accordance with proprietary programs/curriculum. Lite-touch case management services to seniors and adults with disabilities are the main function of the position.  Additionally, RSC is responsible for actively building and maintaining partnerships with community agencies, universities, government, business leaders, law enforcement, and communities of faith to ensure CHP’s clients' access to needed support and services.  The RSC is responsible for compliance and maintaining data on program activities and outcomes as well as all on-time reporting to grantors and funding sources. The RSC is responsible for the implementation and coordination of service delivery to individuals’ families within an assigned geographic area encompassing central Virginia, including Henrico, Farmville, Charlottesville, Lexington, Amherst, and Covington areas.

The ideal candidate should possess effective communication and interpersonal skills, stellar customer service skills, strong problem-solving skills, and the ability to establish effective working relationships with diverse residents, co-workers, and community partners.  Proficient with internet research, and Microsoft Office suite. Extensive knowledge of community resources to support and sustain residents’ self-sufficiency and stability. Should be able to work independently as well as to work within a team-oriented environment.  Previous experience working with senior/disabled populations, case management, benefits and entitlements, and in-home or facility-based services required.

  • High School Diploma or equivalent plus 2 years experience working in the human services/social services field; OR
  • Bachelor’s degree in the human services/social services field plus 1 year of work experience in the human services/social services field
  • At least 1 year of the experience must include experience working in the area of focus delivering Senior Services.
