Hours: 40+ hours includes weekdays and weekends on a rotating schedule

Education and Experience:

●      Possess a Bachelor's or Master’s degree from an accredited college or university in the area of behavioral science, psychology, counseling, social work, or a related field of study and at least two (2) years of experience in the field of residential service programming.

●      Must have strong interpersonal communication skills, public speaking skills, administrative/computer skills and the ability to manage multiple tasks and competing priorities.

●      Preference will be given to candidates who hold a professional license by the state of Georgia (LPC, LMFT, or LCSW).

Position Summary: The Micah’s Promise HSP will be responsible for receiving all intake calls and responding to all intake emails for the girls residential Maximum Watchful Oversight (MWO) program. This position will serve as the liaison to our partnering referral agencies. The HSP will field email and telephone referrals, print and review all relevant documentation, prepare/present each potential referral to the clinical team, make placement decisions and facilitate the placement of accepted referrals. The HSP will also provide some case management support and is responsible for all transition and discharge planning.

Responsibilities:

●      The duties of this position include but are not limited to the following: · Coordinate intakes, staff new residents with the clinical team, and schedule and facilitate the admission process

●      Complete all relevant assessments and paperwork for each new admission in accordance with RBWO/RHC deadlines: Screening/Intake assessment, 7- day Individual Service Plan (ISP), Casey Life Skills assessment, 30- day ISP, and others as needed)

●      Facilitate and provide intake information in Team Review Meetings

●      Provide case management services deemed necessary by HSP Lead

●      Maintain service and transition plan in coordination with therapists and program staff

●      Maintain updated referral, admission and discharge data in GA SCORE /EHR

●      Complete monthly chart/ file audits as well as updating the participant census board

●      Coordinate Participant discharges, including Transition Plan and Discharge Summary paperwork

●      Update daily/monthly logs as needed (Admission Data Log, EHR)

●      Attend weekly clinical team meetings and other administrative meetings as needed

●      Participate in monthly on-call rotation (maintain on call mobile phone, respond to calls within 15 minutes, return to RHC site after hours as needed for emergencies)

●      Attend and actively participate in monthly CSEC MDT meetings (Commercial Sexual Exploitation of Children Multi-Disciplinary Team)

●      Serve as the community liaison with CACG, DFCS, DJJ, Juvenile Courts, Schools, other community partners and outreach organizations

●      Make intentional connections in the community to ensure strong referrals

●      Coordinate appropriate services with community partners

●      Other duties to support program implementation as assigned

●      Ensure all required data collection pertaining to the role is complete and accurate by determined deadlines

Qualifications:

●      Never have been shown by credible evidence (e.g., a court or jury, a department investigation, or other reliable evidence) to have abused, neglected, sexually exploited, or deprived a child or adult or to have subjected any person to serious injury as a result of intentional or grossly serious injury as a result of intentional or grossly negligent misconduct as evidenced by an oral or written statement to this effect obtained at the time of application and evidence of having made efforts to obtain and evaluate references from previous employers · Documentation of at least two professional, educational or personal references that attest to the person’s capability of performing the duties of the position and to the person’s suitability of working with or around children

●      Documentation from a licensed physician or other licensed healthcare professional of a health screening examination within thirty (30) days of hiring sufficient in scope to identify conditions that may place residents at risk of infection, injury, or improper care · Satisfactory preliminary criminal history background check determination and a satisfactory fingerprint records check determination as required by law

●      Clean driving record is essential to fulfilling the job responsibilities

●      Willingness to pursue and participate in continuing education to stay up to date on best practices and regulatory compliance requirements

●      Current evidence of successful completion of a biennial training program in cardiopulmonary resuscitation (CPR) and a triennial training program in first aid which have been offered by a certified or licensed healthcare professional; such programs shall be completed within the first year of employment

●      Participate in all required trainings and show competency in skill areas of training content

●      Must be 21 years of age or older

Position Requirements:

●      Minimal local travel required related to participant and program needs, as well as  occasional travel for training

●      Ability to lift objects up to 25 pounds, work for long periods at a computer, and drive  company vehicle as needed

●      Ability to administer physical restraints as governed by program policies

●      Satisfactory criminal  background and  fingerprint records check, as required by law

●      Clean Motor Vehicle Record

●      Documentation from a licensed physician or other licensed healthcare professional of a health screening examination within 30 days of hiring sufficient in scope to identify  conditions that may place residents at risk of infection, injury or improper care

●      Completion of a biennial training program in  cardiopulmonary resuscitation (CPR) and a triennial training program in first aid which  have been offered by a certified or licensed healthcare professional within the first 30 days of employment

●      Participation in all required trainings required by licensure and organization
