Description

Under the supervision and director of the Director of Managed Care (DMC), Grants Manager (GM) is responsible for a portfolio of prospective and current funders - foundations, corporations, and government agencies. Grants Manager's scope of responsibility extends from research to letters of inquiry to drafting and submission n of proposals. GM also stewards donors of grants, providing timely reports and compliance. As part of the stewardship process GM reaches out to representatives and program directors of funding entities as ambassador of the Health Center.

Job Duties:

1 . Meets with Health Center executives and management to identify funding needs and priorities, to develop proposal strategies and to gather related information.

2.    Works closely with others in Grants team to research and analyze prospective grantors among foundations, government agencies, and corporations; to match the interests and preferences of funding sources to the needs of the health center.

3.    Gathers and/or drafts supp01iing information and data to prepare letters of inquiry, proposals, and applications for funding to all foundation, government, and corporation prospects according to their guidelines for application.

4.    Approaches, develops and cultivates long-term effective working relationships with representatives of private foundation ions, government offices, and corporations in order to promote/negotiate grant opportunities; follows up regularly on grant applications and opportunities.

5.    Works with others on Grants team to utilize appropriate software/database to maintain records on potential and current funders regarding meetings, communications, proposals, grants and rep01is; to utilize appropriate scheduling programs (such as Microsoft Outlook) if not the database, for scheduling and tracking of all LOI' s, proposals, repo1is, etc.

6.    With others in Grants team develops reports on progress status of proposals and grants received, requested, denied and pending; prepares and files periodic management progress reports ·With grant funding sources in a timely manner; monitor' s calendar to ensure timely submission of required information.

7.    Reviews grant utilization accounting reports monthly and flags/follows up on any suspected abnormalities.

8.    Develops and ensures that funders receive comprehensive repo1is in accordance with grant contracts and preferences; coordinates with relevant health center staff to collect pertinent data in a consistent and timely manner; coordinates with the accounting and fiscal staff for needed financial information and data/statistics reports.

9.    Maintains knowledge of programs and services offered at various sites within the organization.

10.    Works with the Communications Director to identify and document health center stories that may be used for funding proposals.

11.    Coordinates need for written and other support materials (photographs, etc.) with the marketing/communications staff and health center staff to ensure appropriate materials to accompany proposals and reports.

12.    Maintains knowledge of best practices in grant fundraising and trends in the nonprofit and FQHC health care sectors through reading, research, participation in seminars and other educational opportunities and personal contacts/memberships in the field; recommends new/updated grant application strategies as appropriate to the health center.

13.    Supervise Grants Coordinator and Policy and Procedure Coordinator

14.    Ensuring that all grant requirements, goals and objectives are met.

15.    Point of contact and responsible for change in scope and grant monitoring including ensuring that Omni is adhering to all grant requirements and grant goals and objectives.

16.    Provides regular progress (or lack thereof) reports to executive leadership and department heads informing them where they are at with relation to grant requirements and holds departments accountable

17.    Responsible for EHB, HRSA grants, communications and be the team lead for the HRSA office site visits.

Communication Expectations:

1.    Ensures effective, professional communications (both verbal and written) within and outside of the health center setting.

2.    Manages effective working relationships with foundation(s) and other funding sources and communicates the health centers strategic priorities and programs when meeting with their representatives.

3.    Prepares reports as necessary for executives and board members, including funder background briefs.

4.    Organizes interdepartmental meetings for key program staff to identify and prioritize needs for funding research and grant submission purposes.

5.    Prepares and/or proofreads all grant LOI's, Proposals, Reports, etc. generated by the Grants Writers.

Additional Duties:

1.  HIPAA compliance - Responsible for maintaining abreast of and in compliance with all HIPAA regulations and requirements. Treats all member information confidential.

2.  Compliance - Ensure compliance with all local, state and federal regulations.

3.   QA/QI - Participate in QA/QI activities and contribute towards the overall performance improvement of the organization.

4.  IT - Required learning and using the EHR/EOHR (Medical Practice Electronic System) and its components. As required by the job functions and highlighted in the Policies and Procedures. These components include NextGen, Health port, PMS, QSI and other electronic features, as they are developed and implemented, as applicable to work environment.

5.  All employees will participate in Patient Centered Health Home Model at Omni Family Health.

Qualifications, Education, and Experience: Education:

Minimum bachelor's degree in communications, English, writing or similar field, and/or grant

writing certification.

Experience:

Minimum 5 years of grant management and grant writing work experience. Strong research skills needed.

License/Certification

Current California driver's license.

Skills/Competencies:

•    Working knowledge of general development practices in a nonprofit environment with specific emphasis on writing/submitting grant applications.

•    Demonstrated skills in identifying sources for funds and successfully receiving funding.

•    Ability to analyze and synthesize health center needs and match grant opportunities to those needs.

•    Very effective oral and written communication skills including ability to be persuasive; superior writing skills including editing for content, style, and grammar.

•    Strong personal computer skills including excel spreadsheets, word processing and presentation software; Microsoft Office and Outlook.

•    The analytical skills, experience, and good judgment to make decisions relative to grant opportunities.

•    A working knowledge and experience in developing long-term relationships with government agencies and personnel, foundations, individuals and representatives of companies that might provide support.

•    Ability to work independently and make mature business decisions.

•    A collaborative work style, with a high level of integrity and excellent interpersonal skills.

•    Knowledge of and ability to work competently with an ethnically diverse staff and client population.

•    Demonstrated ability to perform assigned responsibilities with minimum supervision; to maintain quality control standards; to interpret, adapt and apply guidelines and procedures.

•    Skill in identifying problems and recommending solutions in a clear, concise, and proactive manner, including recognition and resolution of costly errors.

•    Demonstrated ability to set priorities and organize work responsibilities to insure completion of assigned tasks as scheduled.

•    Willingness to commit to the philosophy and goals of Omni Family Health Center.
