SUMMARY OF POSITION RESPONSIBILITIES:

The Youth Mentor / Direct Care is responsible for providing care, supervision, and behavior management to adolescents with behavior issues in a QRTP (qualified Residential Treatment Facility) Staffed Residential Home. In addition to the services provided to the youth, the employees are also required to assist in maintaining a clean household environment, provide household and grounds maintenance, and provide basic office management duties.

Essential Duties:

  • Assisting case manager with assessing clients’ physical and mental wellness, needs, preferences and abilities and using these to develop tailored care plans
  • Working with clients, family and friend support networks and health care professionals to put care plans in place
  • Listening to clients’ concerns and providing intervention as needed
  • Recording clients’ progress including charting referrals, home visits and other notable interactions
  • Evaluating clients’ progress periodically and making adjustments as needed to improve outcomes
  • Serve as a position role model
  • Provide guidance and supervision to all youth in care.
  • Provide youth with daily medication(s).
  • Prepare meals and snack per the posted monthly menu(s).
  • Provide transportation to and from activities, counseling and other services.
  • Make telephone calls to confirm appointments and locate resources, and perform other duties as assigned.
  • Maintain current information, and adhere to, program policies and procedures.
  • Maintain records and statistics as required by the agency, state, and other funding sources.
  • and will perform administrative assignments as requested.
  • Following up with discharged clients to ensure they were satisfied with services, and they are still in good physical and mental health

 Education and Experience:

  • Be at least 21 years of age
  • Minimum High School diploma, Associates, Bachelor's Degree or Masters
  • A minimum of one year working with children (2080 hours).
  • Possess the experience, personal qualities and practice skills to work effectively with professionals from a variety of disciplines both internally and with other organization.
  • Strong organizational and time management skills.
  • Excellent oral and written communication skills.
  • Excellent interpersonal skills.
  • Valid driver's license and proof of insurance.
  • Must be able to pass a criminal background check, which may include fingerprinting
  • Must complete all 16 hours of training to be left unsupervised with the youth.
  • Attend all staff meetings

 Additional requirements include:

  • Flexible schedule and be available a minimum of 32-40 hours per week
  • Reliable transportation, with a current WA, ID, or OR Driver's License and auto insurance
  • Ability to relate to and work with youth that are emotionally upset, and at times hostile.
  • Ability to make independent decisions when circumstances warrant such action
  • Must also fulfill all contract and program training requirements

PHYSICAL DEMANDS: Regularly walk, stand or stoop; occasionally lift, carry, push, pull or otherwise move objects weighing up to 25 pounds; and regularly drives a motor vehicle.

WORKING CONDITIONS:

All staff are required to fully vaccinated, work is performed in a family home like environment.
