POSITION: Reentry Case Manager/Housing Navigator

REPORTS TO: Director of Programs

FLSA STATUS: Non-Exempt

SALARY RANGE: $45K - $50K, Full-Time, 40 hours/week

Position Summary:

The Case Manager will need to have extensive experiences working with individuals that are homeless and/or individuals returning from incarceration. The Case Manager under the direction of the Director of Programs provides client advocacy, case management, benefit establishment, linkage to Mental Health/Substance Abuse services, linkage to stable housing and all other supportive services as needed. Case Managers/Housing Navigators will provide individualized client support by helping each client develop a plan to address their barriers, increase their income, and maintain and sustain permanent housing. As part of the plan, the Case Manager/Housing Navigator will identify each area in which clients will need assistance to accomplish the outlined goals and objectives (i.e., scheduling appointments, applying for public benefits, identifying subsidized housing, etc.)

Reentry Case Manager - RESPONSIBILITIES/STATEMENT OF DUTIES

  • Service a caseload of active participants, all of which will be adults returning from incarceration
  • Provide individual case management sessions with all clients at regularly scheduled intervals
  • Develop individual service plans for all clients that identify barriers to successful reentry
  • Document all client contact and progress, including time and date, type of contact, outcome and plan of action
  • Provide or broker services to holistically address clients’ needs
  • Oversee sessions with volunteer mentors, including tracking contact times, facilitating activities and resolving issues
  • May also perform the duties of employment training specialist and/or job developer
  • Provide regular updates to the Director of Programs.

Reentry Case Manager - MINIMUM REQUIREMENTS

  • Bachelor’s Degree in Social Work (BSW), Psychology, Counseling, Criminology or a related social/behavioral science field
  • Master’s Degree in Social Work (MSW) provides distinct advantage
  • 2-3 years’ experience in delivering case management services
  • Excellent verbal, written and interpersonal communication skills
  • Ability to work effectively with people of diverse educational and cultural backgrounds, from returning citizens to public officials
  • Computer literacy, including knowledge of basic software applications and familiarity with the internet and email communications
  • Knowledge of criminal justice system and/or experience with returning citizens is preferred
  • Ability to travel within city and surrounding communities
