Director, Clinical-Community Partnerships

Background

United Hospital Fund is an independent, nonprofit organization working to build a more effective and equitable health care system for all New Yorkers, one that is affordable and accessible, provides better patient experience and the highest quality of care, and achieves optimal outcomes. UHF’s clinical-community partnerships priority area works to achieve this vision by working at the intersection of health care, public health, social service agencies, and communities to promote a comprehensive approach to helping patients and families live long, healthy, and productive lives.

UHF partners with key stakeholders, including but not limited to health care providers, community-based social service organizations, state and local government, non-health sector stakeholders (e.g., education, economic development), advocates, and New Yorkers with lived expertise. The work aims to develop innovative programming that addresses the individual social needs and underlying systemic racism that contribute to inequalities in health outcomes. The clinical-community partnerships priority area emerged from and encompasses UHF’s ongoing Children’s Health Initiative work which focuses on improving equitable outcomes in early childhood through screening and referral to community supports for individual patient and family social needs. Moving forward UHF’s clinical-community partnership work will also focus on the role of health care in promoting community-wide health improvement, and the cross-agency and cross-sector collaborations that will be necessary to support the transition to a true population health approach in New York State.

UHF seeks a dynamic director for the clinical-community partnership priority area to work closely with the Senior Vice President for Policy and Program. The chosen candidate will manage staff and projects with an annual budget of more than $1 million and will coordinate with internal and external partners to develop and deliver programming focused on achieving equitable health outcomes through collaboration to address the social and systemic barriers to optimal health.

UHF encourages diverse applications from individuals with experience at the intersection of health care and social service delivery, and experience or clearly expressed interest in policies to address social and systemic barriers to health. Ideal candidates may come from health systems or federally qualified health centers or primary care offices that have been working to address the social needs of individual patients. We also seek candidates from community-based organizations or public health departments that have partnered with health care organizations. We seek a diverse pool of candidates with experience that could include direct patient/client support, project or program management, public health operations, or policy/program analysis, among other types of experience at the intersection of health care and social service delivery and social change.

Position Summary

The Director, Clinical-Community Partnerships (CCP) leads a team of program staff and works closely with the Senior Vice President in in all aspects of CCP strategy and programming including development of funding proposals; program/project design, implementation, and evaluation; project management and reporting; research and data analysis; authoring and co-authoring publications and making presentations; convening and other dissemination activities.

SPECIFIC DUTIES:

Responsibilities include, but are not limited to:

• Manage staff of senior program managers and an administrative assistant in the delivery of active projects across the CCP portfolio, including UHF’s extensive Children’s Health Initiative. Provide direction, guidance and mentoring to support high quality work and effective teamwork.
• Research and consult with experts to stay abreast of activities on the ground, in academic and policy literature, an in local, state and national policy developments.
• Research, author and edit presentations, reports, and issue/data briefs and work with communications to maintain website and social media presence in support of CCP priorities.
• Lead CCP strategy development, creating a comprehensive plan consistent with organization wide strategy focused initially on responding to the effects of the COVID-19 pandemic, and increasing the focus on equity in the content and conduct of the CCP towards the goal of seeking to address racism that impacts the health of individuals and communities.
• Represent UHF at meetings, conferences, and when working with stakeholders, and as appropriate, serve as expert resource to media, UHF partners, and other stakeholders.
• Identify and develop new projects, partnerships, and sources of support for the CCP priority area working collaboratively with direct reports, other divisions, and the senior vice president.
• Maintain and build relationships with partners and funders.
• Write grant proposals and reports for funded work. Develop and execute project work plans, manage grant deliverables and expenditures.
• Develop grant initiatives to be funded in part by UHF’s own grant dollars and work closely with the grants manager, direct reports, and senior vice president overseeing and evaluating grants made by UHF.
• Work with fellow directors, senior leadership, and other staff to engage in strategic planning for UHF initiatives, identify opportunities for collaboration and knowledge-sharing across project areas and divisions, and foster UHF’s core values.

EMPLOYMENT STANDARDS:

Minimum Education and/or Experience Required:  Master's degree in health or public policy, public health, health administration, medicine, nursing, social work, or related field (additional professional or lived experience could substitute for an advanced degree); 10+ years of professional experience working in or with health care or social-service organizations or in other relevant roles focused on improving health care services and social supports for historically underserved and disadvantaged groups. Demonstrated experience working independently and as part of a collaborative team. Evidence of progressive responsibility for staff and project management. Strong track record of partnering with outside organizations, including health care providers or social service organizations and other stakeholders.

Knowledge and Ability:  Excellent written and oral communication skills. Strong staff management and interpersonal skills including relationship-building and facilitation skills are essential. Ability and motivation to work both independently and collaboratively as part of a team are required. Knowledge of health care or social service systems, policy and practice on federal state and local levels, and health care or social services financing is required. Exceptional PC and Microsoft Office suite skills.
