Position: Care Services Coordinator, The ALS Association Central and Southern Ohio Chapter

Reports To: Care Services Director, Columbus Office

Job Status: Part-Time, Non-Exempt

Hourly Rate: Commensurate with experience

Benefits: Paid time off and paid holidays – pro-rated, SIMPLE IRA after eligible time is completed

Hours: 20- 25 hours/week

Location: Work from home office and in the Southwestern Ohio community. The candidate would ideally live in Butler or Warren County, but must be in Southwest Ohio.

 

POSITION DESCRIPTION

The ALS Association Central & Southern Ohio Chapter is seeking a part-time Care Services Coordinator in the Southwest Ohio region. Care Coordinator case management responsibilities will include but will not be limited to providing patients, families, and caregivers with information about ALS; coaching individuals on disease management; promoting The ALS Association Central & Southern Ohio Chapter; connecting with local community resources; and linking with local health care providers. Other responsibilities may include establishing and facilitating a monthly ALS education and support group in a centralized location in the region; connecting patients/families with Chapter programs including the grant program and the medical equipment program; working in partnership with the Care Services Coordinators in other areas of the Chapter territory (Columbus, Cincinnati, and Dayton) to meet the needs of patients and their families; attending the University of Cincinnati ALS Multidisciplinary Clinic as a Chapter liaison: collaborating with the Chapter Events and Development team to make connections related to potential corporate sponsors and  individual donor prospects and identifying grant opportunities; increase outreach efforts to area medical professionals, patients and families in an effort to increase the number of patients registered with the Chapter.

 

MISSION OF THE ALS ASSOCIATION CENTRAL & SOUTHERN OHIO CHAPTER

Founded in 1984 and covering 55 counties in the state, the Chapter serves the needs of those living with Amyotrophic Lateral Sclerosis and their families. The Chapter provides case management, supports two multidisciplinary ALS clinics, facilitates support groups, and loans equipment - - all free of charge. The ALS Association is the only non-profit health organization dedicated solely to the fight against ALS, covering all the bases – patient services, public education, advocacy, and research – providing help and hope to those facing the disease.

 

QUALIFICATIONS

  • Minimum requirements: Graduate of an accredited institution with a degree in Social Work (LSW or LISW) and currently licensed in the state of Ohio strongly preferred. An RN with a current Ohio nursing license may also be considered.
  • Three years of social work or nursing experience in acute, long-term care or home care/hospice, with at least one year in hospice or homecare strongly preferred.
  • Ability to assess psychosocial needs of ALS patients and families
  • Knowledge of local community resources
  • Strong communication and teaching skills
  • Organizational and problem solving skills
  • Must be able to work independently, have demonstrated patient care planning and patient teaching skills, and strong written and verbal communication skills.
  • Computer proficiency and a working knowledge of Microsoft programs and Excel required. Some type of database experience preferred, as entering patient information and notes in the database will be expected.
  • Ability to network with area medical providers, healthcare personnel and vendors.
  • Ability to work as a team member with other Care Services Coordinators, Support Group facilitators and the Care Services Director.

 

 

ADDITIONAL INFORMATION

The ideal candidate will live central to the Southwest Ohio area, potentially in Butler or Warren County. This is an off-site, part time job, with the employee working 20 to 25 hours per week. This is a work at home position, and a reliable high speed internet connection in the employee’s home is required. The employee will be required to have an appropriate, confidential space in their home to set up a home office area, with space to accommodate a desk. The chapter will provide a laptop and desk phone, a scanner/printer and a two drawer locking file cabinet. It is important that the job candidate recognize the importance of maintaining absolute confidentiality of all information pertaining to patients, families, and staff.  Adherence to all HIPAA rules is mandatory. It is expected that the candidate will display a positive, professional image and attitude in all relationships with patients, families, peers and in the community. It is also an expectation that the candidate will attend the Cincinnati and Dayton Walk to Defeat ALS and other local events, representing the Chapter when needed.
