Help Veterans Move from Housing Crisis to Lasting Stability

The Center for Veterans Issues, Inc. (CVI) seeks a compassionate, resourceful, and highly organized Case Manager to support Veterans and their families through the Supportive Services for Veteran Families (SSVF) Program. This position plays a vital role in helping Veterans experiencing homelessness or a housing crisis obtain safe, stable housing and build the skills and connections necessary for long-term independence.

Through comprehensive case management, housing counseling, financial guidance, advocacy, and coordinated referrals, the Case Manager helps Veterans overcome barriers and move forward with dignity, security, and hope.

What You’ll Do

Provide Comprehensive, Veteran-Centered Case Management

You will assess each participant’s needs, strengths, barriers, and available resources, then provide individualized case management and supportive services. You will develop and monitor Housing Identification and Stability Plans and Single Coordinated Care Plans, execute participation agreements, maintain accurate case files, and create thoughtful discharge plans that include appropriate referrals and continued support options.

Promote Housing Stability and Independence

You will provide housing stability counseling and connect participants with rental assistance, rent subsidies, affordable housing, and other housing-related programs. You will help Veterans understand fair housing protections, landlord-tenant responsibilities, lease requirements, rent delinquency, home maintenance, and client rights. Your guidance will help participants make informed decisions and develop the skills needed to obtain and maintain stable housing.

Connect Veterans with Essential Services and Resources

You will build strong partnerships and connect participants with VA programs, County Veteran Service Officers, shelters, Grant and Per Diem programs, safe havens, local Veteran organizations, and nonprofit service providers. You will also coordinate referrals for vocational rehabilitation, employment, education, healthcare, childcare, transportation, legal assistance, protective-payee services, emergency resources, daily-living support, and direct financial assistance.

Provide Financial Counseling and Assistance

You will help participants understand their income, expenses, and financial obligations, and help them develop realistic personal budgets. When appropriate and authorized, you will prepare requests for temporary financial assistance following approval from the Area Manager or Director and ensure all supporting documentation meets program requirements.

Maintain Accurate, Timely, and Audit-Ready Records

You will document participant assessments, contacts, services, referrals, progress, and outcomes accurately and promptly. You will maintain complete participant files and enter required information into HMIS within established timeframes. You will follow CVI policies, grant requirements, confidentiality standards, and documentation procedures to ensure records remain compliant and audit-ready.

What Success Looks Like

Success in this position means every participant has an updated Housing Identification and Stability Plan and Coordinated Care Plan within 30 days of intake. All case files, participant documents, and HMIS records are complete and accurate, with client records updated within 48 hours of service delivery.

At least 80% of participants achieve their housing stability goals within the planned timeframe, 90% are connected with at least one appropriate resource within 30 days of intake, and 85% develop and follow a personal budget plan. Your files remain organized and audit-ready, with no compliance issues identified during internal or external reviews.

What You Bring

You have a bachelor’s degree in social services and at least two years of experience providing case management, life-skills education, assessments, and counseling services. An associate degree may be considered with four to five years of directly related experience.

You communicate clearly and professionally, maintain accurate records, protect confidential information, and build productive relationships with participants, landlords, service providers, and community partners. You are flexible, composed under pressure, culturally responsive, and able to work effectively with individuals from diverse backgrounds and with complex service needs.

Experience serving Veterans and individuals affected by homelessness, substance use disorders, or co-occurring disorders is strongly preferred. Knowledge of the Veteran recovery community, housing resources, and community-based services is highly valued.

This position requires a valid Wisconsin driver’s license in good standing and current automobile insurance. The Case Manager must also be prepared to provide professional, equitable, and nonjudgmental services to eligible participants with varied backgrounds, including individuals with criminal histories, registered sex-offender status, or histories involving violent offenses, consistent with program requirements and safety protocols.
