REPORTS TO: Vice President of Programming

LOCATION: Weymouth, MA and Hull, MA

SUMMARY: Wellspring Multi-Service Center is seeking a skilled and compassionate Program Case Management Coordinator to oversee the coordination and delivery of services and programs to clients and students. This key role supports Wellspring’s mission of empowering individuals and families by providing structured support and strategic case planning within a collaborative team environment.

RESPONSIBILITIES:

·       Conduct triage, intake, and case management assessments for individuals seeking services

·       Provide individual and group counseling for clients experiencing physical or emotional challenges

·       Collaborate with Case Management Team Members to develop and monitor service plans and ensure quality care

·       Evaluate client progress and adjust plans as needed to support their overall well-being

·       Coordinate with Cross-Department Management Team to align internal services and optimize resources for clients, students, and community members

·       Integrate Wellspring programs and services to serve all populations at Wellspring

QUALIFICATIONS:

·       Degree in Social Work, Counseling, or Mental Health Services

·       Minimum of 5 years' experience in mental health and human services delivery

·       Strong clinical skills with the ability to assess client needs and guide agency response

·       Experience in mental health counseling and diagnostic assessment

·       Knowledge of and experience with program development

·       Ability to be flexible, creative, and collaborative in a fast-paced and evolving environment

·       Excellent verbal and written communication skills

·       Strong computer and technology skills

·       Translation or multilingual abilities are a plus
