JOB SUMMARY

Responsible for all aspects of clinical social work services within the Congregate Family Program. Ensuring that quality housing services are provided effectively and in accordance with guidelines established by CHI, DHS, OTDA, and other funding sources as appropriate.

Positions available in the Bronx and Brooklyn.

ENTRY LEVEL QUALIFICATIONS AND REQUIREMENTS

  • MUST BE FULLY VACCINATED AGAINST COVID 19
  • Education: MSW or LCSW from an accredited university or related field of study required
  • Experience: 2-5 years’ experience providing management services.
  • Certificates or Licenses: Valid driver’s license
  • Insurance: Valid automobile insurance
  • Communication: Excellent verbal and written communication skills.  Proficiency in English required. Bi-lingual (Spanish) is a plus.
  • Computer Skills: Basic competency in MS Windows, MS Office, CARES and on the Internet.
  • Math Skills: Must be excellent.
  • Physical Performance: Ability to tour properties, walk distances, climb stairs, etc.
  • Reasoning Ability: Ability to prioritize, make appropriate decisions and judgment calls.
  • Other Skills: Ability to work independently or as part of a team, be highly organized, assume responsibility, work well under pressure, meet deadlines, multitask, be flexible, and maintain a high level of attention to details.  Must be even-tempered, have good negotiation skills, and the ability to meet unexpected deadlines.  Effective time management skills.

JOB-RELATED DUTIES – ESSENTIAL FUNCTIONS

These duties are essential and specific to the successful implementation of this position.

Under the direction of the  Team Leader, the Senior Case Manager will work as part of an interdisciplinary team. The Senior Case Manager will supervise the overall running of the Case Managers for their site.

  • Ensure that the program is providing quality services and are compliant with program requirements.
  • Promotes sound social work practices through case supervision, including case consultation, case conferencing, and case file reviews in a multiple disciplinary approach for the highest quality of care and housing placement success.
  • Work from a trauma informed, strength based, client centered perspective to independently conduct assessments and psychosocial treatment
  • Have knowledge of medical and mental health diagnosis, disabilities and medications
  • Manage a case load. Complete notes, completes and update psychosocials, review service referrals and ILPs in CARES.
  • Provide mental health/substance abuse workshops, onsite support groups and group counseling
  • Provide and coordinate mental health services with on-site staff and make referrals to other agencies.
  • Ensure aftercare services for clients after discharge.
  • Provides crisis intervention and stabilization as appropriate. May required after hour or weekend follow up to emergency situation.
  • Provide in-service training to case managers and other staff. Inform the Assistant Program Director of deficiencies that require additional staff training.
  • Ensures the timely and correct submission of all internal and external reports in the manner required by DHS, and/or CHI (housing logs, client contact sheets, progress notes, authorization forms, ILPs, referrals, admission and discharge, etc.).
  • Adheres to proper use of company resources (cell phones, office supplies, internet, etc.).
  • Maintains appropriateness at all times.
