About Encore: For over 40 years, Encore Community Services has provided care to the elderly of the Clinton, Times Square, and Midtown communities, so that they may live with dignity and decency in a safe and caring environment. Encore provides a comprehensive array of programs for older New Yorkers including supportive and affordable housing, home delivered and sit down meals, case assistance, education, recreation, and friendly visiting. The organization takes a “no wrong door approach” to ensure Seniors are able to access all of their neighborhood and other entitled resources so they may age in place comfortably for as long as possible. Encore is at an exciting inflection point in its development. The organization is working to build off of a rich history to deepen its services and scale the work of giving back to seniors. Encore is keenly aware that the demographics of Older New Yorkers is rapidly evolving. Therefore, Encore must also evolve to ensure the health, mental health, and economic stability of New York’s diverse and vibrant senior community. To this end, a major initiative is the refinement and expansion of the organization’s social services and program activities department.

About Position: The Service Coordinator is directly responsible for assisting Residents and coordinating services at Encore’s Single Residence Occupancy (SRO) program, called Encore 49. This program serves formerly homeless senior citizens, many of whom have a mental illness and other disabilities. Service modalities include Trauma Informed Care, Harm Reduction, and Motivational Interviewing. Job duties include conducting needs-based assessments, community outreach, providing case management services, supporting housing retention, connection to income resources and/or public benefits, attending meetings/trainings within the community and collaborating care with service providers and multi-disciplinary teams to enable Residents to live independently and age in place for as long as possible. The Service Coordinator is responsible for organizing and/or facilitating health, wellness, and educational programs that are tailored to Resident needs in order to improve their wellbeing and foster a strong sense of community. The Coordinator will maintain existing community partnerships and establish new relationships with the aim of expanding opportunities and resources available to Residents of Encore 49.

Responsibilities:
• Ensure Residents’ voices are heard, and their ideas play a prominent role in programming;
• Assess services needed by Residents through surveys, visitations, and group meetings.
• Identifying and coordinate services for the unique needs of the Residents and utilize screening and assessment tools to make those determinations;
• Provide Residents information on specific services and/or program providers, giving several options whenever possible;
• Encourage the Residents and/or family members to arrange for services that will be helpful towards maintaining and encouraging their independence;
• Coordinate and monitor the services and activities of providers to Residents to assure quality of care and cost effectiveness;
• Advocate for residents per their wishes;
• Maintain up-to-date files on service providers in the community;
• Maintain and submit accurate and updated records on all Residents to include the services they receive;
• Develop support groups and workshops in response to needs and facilitate /co-facilitate these groups on a regular basis;
• Assist in relocating residents as needed and securing emergency housing;
• Support team members by serving as a resource for analyzing and solving problems and staying abreast of current issues and theories within the field;
• Responsible for implementing the Service Coordinator program and completing annual Service Coordinator credentials;
• Attend orientations, trainings, education programs, staff meetings, community meetings, conferences and workshops as requested and applicable to meet the needs of the position;
• May provide training and mentorship to team members and community stakeholders regarding best and promising practices in relevant service models and practices;
• Expert written, oral, and interpersonal communication skills;
• Clearly document all client interactions along with required eligibility and demographic information;
• Perform duties in a professional manner by maintaining the confidentially of all information and by participating effectively within and across teams;
• Must complete annual CEU requirements to maintain Service Coordinator Credential;
• Perform job responsibilities in accordance with the Social Work Code of Ethics;
• Perform all other duties as assigned.

Qualifications:
• Sensitivity and empathy for Residents, colleagues, and community partners;
• Two years’ experience and BA in related social service field;
• Proven experience with the seniors who are formerly homeless, and/ or those with a mental illness diagnosis;
• Strong clinical skills;
• Expert written, oral, and interpersonal communication skills;
• High level of competency working with Microsoft Office suite and cloud-based applications;
• Strong time-management and prioritization skills;
• Experience working both independently and, in a team-oriented, collaborative environment.

• Knowledge in AWARDS database a plus;

Preferred Qualifications
• Bilingual in a language represented in the resident community (Mandarin Chinese, Vietnamese, or Spanish) is a plus.
