Department Assisted Living Community Reports to Chief Program Officer and Assisted Living Administrator Position Purpose Provides direct nursing and wellness care to the Assisted Living Community (ALC) residents aka Villagers and supervises the day-to-day clinical activities performed by the licensed practical/vocational nurse and certified nursing assistants and CMAs in accordance with current federal, state, and local regulations and guidelines and established facility policies and procedures. The RN Manager position will be the liaison between the villager, villager family, and designated pharmacy and Villager medical providers.  The RN Manager is an integral part of both the Assisted Living Community and the Health Services team  to ensure the villagers receive holistic care.

Required Qualifications

Prior experience in long-term care preferred but not required.

A Nursing degree from an accredited college or university with at least 4 years of nursing experience.

Current unrestricted license as a Registered Nurse (RN) in Georgia and active CPR certification.

Two (2) years’ experience working with adults with developmental disabilities and med/surg nursing preferred.

Have proficient computer skills with working knowledge of programs such as Word, Excel, Outlook, etc.

Must be at least 21 years of age, have a valid Class C Georgia Driver’s License, proof of vehicle insurance, and meet company policy regarding MVR requirements.

Must meet state requirements regarding a GBI and FBI criminal history record check which requires fingerprinting.

Must undergo and pass a drug screening and screening for Tuberculosis.

 

Major Duties and Responsibilities

Directs the daily activities of the ALC and personnel- licensed practical/vocational nurse and certified nursing assistants in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures.

Follows the nursing model to promote holistic care for the residents.

Participates in the admission, discharge and transfers of residents in collaboration with Admissions Team, as required.

Assesses changes in residents’ status, notifying the physician and resident’s family or representative and documenting accordingly.

Prepares and administers medications as per physicians’ orders and observes adverse effects as indicated.

Schedules follow up appointments for residents and transportation needs as indicated.

Performs rounds to ensure resident needs are being met and personnel are performing their assigned duties.

Collaborates with other members of the interdisciplinary team as needed to ensure residents’ needs are holistically met.

Participates in management team meetings as indicated to discuss census changes, resident changes in status or care need, or complaints or concerns.

Evaluates work performance of nursing and clinical personnel and implements discipline according to operational policies and as directed.

Reports any grievances, incidents or unusual occurrences to the Administrator, CPO and Compliance Officer, and others per policy and participates in the investigative process as needed.

Ensures that there is adequate stock of medications, supplies, equipment and notifies appropriate personnel of needs.

Initiates, reviews and updates care plans as required.

Promotes teamwork, mutual respect, and effective communication.

Participates in the survey process when inspection surveys arise.

Participates in QAPI and other CQI activities as needed or assigned.

Ensures that infection control principles are followed and helps correct deficiencies in practice as noted.

Reviews all incidents and accidents and the 24-hour report daily to ensure prompt follow up and appropriate interventions that are carried through to the Service Plans, or other areas as needed, for assigned villagers to reflect current and changing needs.

Ability to work with minimal supervision, take initiative and make independent decisions.

Is the primary liaison between Annandale’s selected physician group and pharmacy. Complete processing of medication order, re-order, delivery, and verification processes.

Serve as the Nurse On Call (NOC) nights and weekends, at a minimum of one week per month

Oversees the Villager’s assigned to Self-Administration of Medication (SAM) program to include assessments and monitoring

Assists in overseeing and managing the proxy care program.

Leads the organization and implementation of the on-site Immunization Clinic

Ability to read, write and speak English. Ability to communicate at a high level with  Villager families regarding all clinical care

The employee frequently is required to sit, stand, and walk.  Employee is occasionally required to climb or balance; stoop, kneel, crouch, bend, or crawl.  Employee is required to lift up to 25 pounds and/or move up to 75 pounds (with assistance).  Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. Employee may be required to transport (drive) and assist Villagers in and out of vehicles. Employee is required to perform CPR/First Aid.
