JOB SUMMARY:

The Service Implementation Manager (SIM) reports directly to the Director of Programs. The SIM will ensure that the program and team aligns with the mission and values of the organization and that the activities and day-to-day functions are being performed to the highest standards. Excellent written and verbal communication skills and strong interpersonal skills are required. Must be highly organized and be able to manage multiple tasks while maintaining composure.

The SIM will be responsible for leading a Community Support team comprised of a Support Specialist, PCP Specialist, an Employment Specialist, Community Support Managers and Community Support Professionals to assist, and support, adults with developmental disabilities toward the development of skills, and achievement of personal goals as outlined within the Person-Centered Plans.

The SIM will oversee the programming for 40-50 individuals in services in what is called “a community group”.

ESSENTIAL FUNCTIONS:

Supervisory Duties

• Directly supervise Support Specialist, PCP Specialist, and Community Support Managers in all aspects of their positions including the person-centered planning process, goal implementation, job development and job coaching, supervision of direct reports, transportation, and both community-based and in-house programming.

• Collaborate with the PCP Specialist and Support Specialist as it relates to service needs, Service Implementation Plan (SIP) development, and service funding.

• Work closely with Director of Programs to oversee and ensure accurate completion and submission of routine paperwork including attendance, service

documentation, PCP review, goal progress, etc. within the SIM’s “community group.”

• Facilitate weekly team meetings with supervisees to discuss programming (employment, progress toward goals, new goals, issues, community-based and in-house schedules, staffing)

• Work with Director of Programs to ensure quality service provision by addressing and resolving conflicts and barriers to services and reinforcing successes in achieving individuals’ outcomes and goals with the “community group.”

• Conduct 1:1 monthly supervision meeting with supervises to review performance related topics, provide constructive feedback, and assist with problem solving to help those in services achieve their goals. Promote and reinforce Opportunities, Inc.’s Mission and Values throughout all supervisory exchanges.

• Working with the Director of Training and Quality Assurance to provide initial and/or ongoing staff training as required on topics such as: behavioral plans, abuse, and prevention, iCareManager, incident reports, identifying needs of people served, job development and attendance.

• Ensure that all staff providing direct support are trained on the Person-Centered Plan of those in services.

• Proactively address performance problems with Human Resources Director to effectively resolve issues.

• Conduct employee performance reviews after the probationary period and on an annual basis.

Intake Process

• Assist the Director of Programs as needed to review applications and referrals from various agencies/schools for potential new participants.

• Assist with the admissions process and any necessary paperwork.

Person-Centered Planning (PCP)

• Review PCP plans regularly with Support Specialist, PCP specialist and CSM’s to ensure that needs and preferences are being met as it relates to the daily provision of services.

• Attend PCP meetings as needed and support all PCP team meetings to be led by the person in services.

• Ensure communication and feedback flow through the team from CSP’s to Director of Programs as it relates to participants’ goals and/or behavior issues.

• Ensure the monthly tracking of progress on individual PCP goals is completed by PCP Specialists.

• Monitors service delivery as related to the person’s individualized goals and precipitates changes as needed.

Interdisciplinary

• Work in conjunction with the behavioral consultant to develop and implement behavioral plans and guidelines, as needed.

• Work with CCS agencies, as needed, to support people in services in meeting their goals.

• Work with Director of Programs to develop partnerships with parents, teachers, CCS agencies, DORS and other community members and stakeholders to facilitate awareness about Opportunities, Inc.’s programs.

• Assist CSM with transportation oversight as needed.

Communication

• Keep all staff informed of new developments regarding the people we support, as well as other important organizational information.

• Work closely with other SIM’s and “community groups” to ensure smooth daily operations.

• Keep open lines of communication with parents and all agencies, within SIM “community group,” that work with the people who choose Opportunities, Inc.’s services.

• Ensure that direct support staff communicate effectively with all entities of a support team as necessary.

• Develop an email and cell phone database of parents/providers for quick communication.

Quality Assurance

• Schedule and participate in regular file reviews.

• Complete Incident Reports and Agency Investigation Reports with team

• Prepare for and participate in quarterly Standing Committee meetings.

• Assist Support Specialist Attendance Review within agency’s internal documentation for billing purposes.

• Assist in the maintenance of the Diagnosis Book and ensure staff review and sign when updated.

• Work with the Director of Training and Quality Assurance to establish a review of staff training files and ensure that new staff receive all required trainings prior to working independently with people in services.

Other

• Coordinate and participate in the process of discovering person-related needs, including, but not limited to ADLs, employment, advocacy.

• Coordinate the maintenance and reporting of Opportunities, Inc. technology, and other equipment utilized by supervisees.

• Complies with all agency policies and procedures to approve and submit timesheets, mileage reimbursement, and expenses.

• Monitors staff expenditures, and the implementation of overtime, ensuring that any overtime needs are known, and approved by the Director of Programs, ahead of time.

• Participate in the development, review, and implementation of policies and procedures that govern Opportunities, Inc.’s programs.

• Ensures all individuals are served in accordance with approved funding on a weekly basis.

• Participates, and coordinates with other management team members, contributing ideas and efforts to ensure comprehensive services are provided effectively and efficiently.

QUALIFICATIONS:

• Bachelor’s degree in counseling, Social Work, Business or related field required, master’s degree preferred.

• At least 2 years’ experience working with adults with intellectual and developmental disabilities in a leadership role.

• Prior management experience required.

• Abreast with Montgomery County and/or Prince George’s County activities and resources

Salary: $60,000
