Job Summary:

Penny Lane Centers’ Transitional Housing (TH) Program provides support services to Transition Age Youth (TAY) between the ages of 18 to 24 years of age, who are homeless or at risk of being homeless.  Case Management is a collaborative process of assessment, planning, facilitation, care, coordination, evaluation, and advocacy for options and services to meet an individual’s (and Family) comprehensive needs through communication and available resources to promote safety, quality of care, and cost-effective outcomes. Case Manager is responsible for supervision, protection, and care of residents, at all times. Case Manager will assist participants in obtaining additional services needed including but not limited to benefits, outreach linkages, substance use disorder treatment, and mental health/health services. Case Manager will also provide residents with referrals to educational services, employment, and other community resources, as needed. Case Manager will be expected to meet with residents in their units, to ensure the unit is a safe and clean environment and to check for any repairs that may be needed.  As well residents may meet in the Case Manager’s office for formal meetings.

The Case Manager is an energetic self-starter who can function comfortably in a team environment or independently and relates well to co-workers and representatives of other agencies and County representatives. Case manager should have the ability to plan, facilitate, coordinate, and evaluate services that will directly impact the likelihood of client’s success, and must always make the needs of the client their top priority and must be mindful of the client’s values, beliefs, and culture.  Case Manager must provide participant with access to resources, tools, and services to make an informed decision.

This position requires flexibility and the ability to manage time and multiple tasks, as well as problem solve. The individual in this position must be able to assume a wide range of responsibilities, provide timely responses to requests and deadlines and works well under pressure.

Requirements:

  • Preferred: Bachelor's Degree in a related field such as Social Work, Psychology, Education, Counseling, Criminology or Social Welfare, Child Development, or other relevant fields especially with regards to TAY homelessness and vulnerable populations and/or social services.
  • Must have a minimum of 2 years’ experience working with vulnerable populations and at-risk youth in a public or private child welfare social services setting and/or specific experience working with TAY and homeless population.
  • Valid California driver’s license; current automobile insurance; have and maintain a clean driving record acceptable to the organization’s insurance company; have immediate access to his or her vehicle during work hours.

Position Locations available: North Hills, CA and North Hollywood, CA

Position Status: Full Time, Overnight and Intermittent available.

Salary Range: $20.04 up to $28.50 per hour

Additional $1.00 per hour overnight shift differential.
