HOMELESS OUTREACH CLINICAL COORDINATOR JOB DESCRIPTION SUMMARY

We are seeking a highly disciplined candidate who shows courage, empathy to serve and leadership; a thoughtful communicator and problem-solver, relationship-oriented and committed to strengthening our community.

Under general direction, the Homeless Outreach Clinical Coordinator is responsible for coordinating, planning, and supervising the delivery of case management and the analysis of Centro’s Homeless Outreach Program.

The Clinical Coordinator will engage and provide outreach to people experiencing homelessness with severe mental illness, substance use disorders or medical concerns residing on the streets and at encampments in the downtown area.

This position will work closely with City of San Antonio Department of Human Services, SAPD, and community partners and will participate in coordinated downtown area outreach efforts.

This position is “grant funded” and is likewise subject to at-will employment status and can be terminated at any time for any reason.

 

PRIMARY RESPONSIBILITIES

  • Supervises trains and directs assigned staff to facilitate effective and responsive case management to clients in grant and/or privately funded homeless outreach programs and activities.
  • Supervises, trains, and directs assigned staff to implement homeless outreach-related programming and activities.
  • Provides clinical coordination and assessments and case management to clients to assist them with housing navigation, employment and stabilizing their mental health and medical situations.
  • Promotes, educates, and supports program goals and objectives. Drives continuous improvement of homeless outreach program services and activities based on current participant and community needs.
  • Provides oversight of counseling services to individuals in homeless outreach-related programs and activities.
  • Serves as liaison between Centro and community boards and committees.
  • Analyzes complex data, policies, practices, systems, procedures, and develops written summaries on findings and recommendations. Prepares management reports and delivers presentations.
  • Monitors program performance relative to goals and objectives and takes appropriate action, when indicated, to ensure compliance with grant and program performance requirements.
  • Reviews monitoring outcomes and conducts necessary and timely follow-up and supervision for correction with appropriate staff.
  • Supervises, develops, evaluates, counsels, and disciplines personnel according to established Centro policies, procedures, and guidelines.
  • Performs related duties and fulfills responsibilities as required.

 

EDUCATION AND JOB REQUIREMENTS

  • Master's Degree in Social Work from an accredited university
  • Two (3) years of experience in social work or related experience. Two (3) years of supervisory experience in social work or related experience.
  • Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) from the Texas Department of State Health Services
  • Strong verbal and written language skills in English, and Spanish a plus
  • This position requires a valid Class "C" Texas Driver's License OR valid driver's license from another state with the ability to obtain a valid Class "C" Texas Driver's License within 30 days of becoming a Texas resident. Use of your own personal vehicle may at time be required and proof of Liability Insurance will be required.

 

WORK ENVIRONMENT

 

  • 80-90% of work will be performed outside of an office environment and the remaining 10-20% of work will in an office environment at the Centro Operations Headquarters. The Clinical Coordinator exercises direct supervision over assigned staff and conducts research and analyses of program activities and performance. Some reporting and data entry may be performed from home with web-based programs and communications.

 PREFERRED QUALIFICATIONS

  • Two years of experience with street outreach and working with the homeless population

  • Unless otherwise stated, applicants are permitted to substitute two years of related full-time experience for one year of higher education or one year of related higher education for two years of experience in order to meet the minimum requirements of the job.
  • Applicants selected for this position must receive satisfactory results from pre-employment drug testing and background checks. If required for the position, a motor vehicle record evaluation, and additional background checks may be conducted.

  • Ability to work in a collaborative environment building organizational and stakeholder relationships and planning
  • Ability to maintain good relationships with stakeholders focusing on providing excellent customer services and presenting a positive image of Centro San Antonio in all community and stakeholder interactions.
  • Ability to maintain a working environment which encourages mutual respect and congenial relationships between employees and is free from all forms of harassment
  • Knowledge of downtown San Antonio ecosystem a plus
  • Knowledge of the theory and methodology of direct supervision and management of human resources
  • Skill in utilizing a personal computer and associated software program
  • Skill in creating and using computer spreadsheets and graphical presentations
