Your Future Team

Our San Diego office is seeking Social Workers/Case Managers for our local Transition Age Youth Services including supportive housing programs.  My Brother’s Keeper Transition Age Youth programs offer assistance that is client-centered, trauma-informed, and reflective of best practice standards in the field.

Your Impact

We serve youth ranging in age from 18-21, our multi-faceted approach ensures youth receive the guidance to support housing, employment, education, wellness, and other needs they may have for a successful transition. Our Case Workers are responsible for offering participants a safe and welcoming environment to support their transition to independence and self-sufficiency. This position will be responsible for providing crisis intervention, case management, on-site community engagement activities, and daily programming operations at various hours and days. This position will provide a strengths-based, individualized approach to assist our participants in reaching their personal goals and will be responsible for assuring maintenance of a safe, clean, and nurturing living environment for the residents.

Your Skills and Duties

  • Conduct client-screening interviews and assess service needs via phone and face to face interview
  • Assessment and case planning on each participant residing in scattered site apartments
  • Provide crisis intervention, situational and group counseling to residents and phone clients
  • Coordinate case planning with residents, clients and collaborating agencies
    Provide information and referrals to other community resources and services
  • Keep well-written, concise case notes to include all pertinent information for each participant using County and State mandated tracking systems
  • Maintain records and case files for all participants
  • Follow up on discharged/aftercare clients
  • Communicate effectively with Program Directors, co-workers, participants, families, and other service providers
  • Provide a secondary on-call option during specified times
  • Ensure residents maintain safe and adequate living environment in accordance with California Community Care Licensing standards
  • Work independently and as a team member to create innovative ways to meet contract goals and provide a high level of service that is engaging and meets the needs of the participants
  • Develop and maintain community outreach regarding the Transitional Housing Programs (THP+, and THPP-NMD) and Transitional Housing & Youth Development Department, including volunteers and local businesses

Education & Work Experience

  • Must be 21 years of age or older
  • Bachelor’s degree in social science, psychology or a related field is required, Master's degree preferred
  • Experience working with young adults (ages 18-24) required
  • Familiar with the operations of a transitional living program
  • Experience with Independent Living Skills issues preferred
  • Basic computer knowledge and skills
  • Must be able to work as part of a team
  • Must be self-aware, flexible, adaptable and teachable
  • Valid CA Driver's License
  • Current CDL H6 print-out (must be dated within 30 days of application.)
  • Pass pre-employment drug testing
  • Criminal record clearance by being fingerprinted prior to reporting to work and signing a statement regarding criminal convictions
  • A physical exam and negative TB test results
  • Ability to adequately observe participants activities, enforce safety regulations and apply appropriate policies and procedures
  • Current CPR and First Aid certification required and must be from one of the following certifying organizations: American Red Cross, American Heart Association or American Safety & Health Institute
