Description

Developmental Pathways is hiring Active/ Enrollment Case Managers. Our Case Managers will provide comprehensive case management services to improve or maintain each person’s abilities to function in the community. They will assess, monitor, and follow up with person’s ability to meet basic needs in moving towards a meaningful life and will be responsible for all clinical documentation and will verify eligibility for Medicaid services. They will also develop, monitor, and assure implementation of the waiver Service Plan (SP). In addition, they will facilitate and coordinate Interdisciplinary Teams (IDTs) and execute referrals for community supports and services. Lastly, our Case Managers will coordinate support in emergency and crisis situations.
This is a hybrid position working 40-hours/week remotely and coming into our headquarters office in Aurora, CO and/or off-site locations at least once a week and as needed.

Starting Salary and Benefits
$21.90 - $23.54 / hourly or $22.95 - $24.59 / hour for Bilingual Case Managers

Additional Responsibilities 

  • Assure services and supports identified in the SP are coordinated among various providers for continuity of services and funding for individuals served
  • Ensure timely initiation and implementation of service development and coordination processes (SP, annual reviews, and enrollments)
  • Complete annual and initial (as assigned) waiver-specific applications [such as CES applications], as component of annual review and/or enrollment SP
  • Ensure the accuracy and timely entry and distribution of written SPs, addendums, and other support documentation to all necessary parties, including, but not limited to providers, individuals, and family members
  • Complete accurate case documentation (log notes), including targeted case management (TCM) activities, within established timelines
  • Complete mandatory and needs-based assessments to identify individual strengths, needs, concerns, and preferences through interviewing, observing, and utilization of standardized tools
  • Establish person-centered goals and a plan of care (service plan) with the individual and their natural supports/family members along with other members of the IDT
  • Monitor the ongoing provision of and need for services by assessing the delivery and quality of services and interventions provided by external providers; evaluate effectiveness of services delivered (as authorized by the service plan) and assure identified outcomes are met; advocate for individual service needs
  • Provide case management services and supports to temporarily assigned individuals; support with department and team coverage as needed

Qualifications

Requirements

  • Bachelor’s degree
    • OR 5+ years of professional experience working specifically with the long-term services and supports (LTSS) population
    • You will be required to provide a copy of your transcript(s) (official or unofficial) or a copy of your diploma(s)
  • Ability to comfortably learn and navigate technology (online databases, software systems and programs)
  • Excellent oral and written communication skills
  • Desire and dedication to provide the best customer service
  • Strong organizational and negotiation skills
  • Personal vehicle in good operating condition for use during work
  • Valid Colorado motor vehicle license/proof of adequate motor vehicle insurance
  • Meet our driving requirements - no suspensions and no more than two (2) tickets in the past three (3) years

Preferred

  • One (1) year of experience in human services or a related field is strongly preferred
  • Working knowledge of the developmental disabilities system, rules and regulations
  • Specific working knowledge of the intellectual and developmental disability system, rules and regulations
  • General knowledge of rules and regulations for Colorado Medicaid waivers and Colorado Department of Public Health and Environment

Additional Requirements for Bilingual Case Managers

  • Able to speak, write and interpret fluently in English and Spanish
  • Ability to complete all typical case management responsibilities in Spanish
  • Support with written translation of internal documents for use by stakeholders, as appropriat
