The Certified Community Behavioral Health Clinic (CCBHC) model alleviates decades-old challenges that have led to a crisis in providing access to mental health and addiction careAs an integrated and sustainably-financed model for care delivery, CCBHCs:

  • Ensure access to integrated, evidence-based addiction and mental health services, including 24/7 crisis response and medication-assisted treatment (MAT) for addiction.
  • Meet stringent criteria regarding timeliness of access, quality reporting, staffing and coordination with social services, criminal justice and education systems.
  • Receive funding to support the real costs of expanding services to fully meet the need for care in their communities.

 

RESPONSIBILITIES:

  • Provide direct assessment, evaluation and counseling clinical services including evidenced- based individual, group, family, and crisis intervention as clinically indicated.
  • Ensure rapid and easy access, in part by scheduling sessions according to client availability.
  • Be an integral part of the CCBHC multidisciplinary team, including family care planning and collaboration.
  • Conduct warm hand-off referrals to appropriate services and supports.
  • Assist administrative staff in collection of information for program effectiveness.
  • Maintain accurate clinical records in compliance with all city, state, and federal regulations.
  • Adhere to managed care requirements including consulting with care coordinators, obtaining treatment authorization, and providing necessary documentation.
  • Participate in all training programs for the Whole Family Health Model.
  • Attend individual supervision with clinical supervisor.

SKILLS AND KNOWLEDGE:

  • Highly skilled and knowledgeable in treating mental illness, substance use, and co-occurring disorders with a strength-based, whole health approach.
  • Crisis intervention response, support and follow up.
  • Ability to effectively communicate and interact with clients, colleagues, public, other agencies and their staff, and third-party insurance personnel.
  • Cultural competence with Latino and other culture.

 

QUALIFICATIONS:

Master’s degree in social work, counseling, education or other relevant human service profession, with two years’ experience in children and family services.  Ability and willingness to do home visits. Ability to work independently, as well as contribute to a team. Very good interpersonal/communication and strong clinical skills.

 

Bilingual Spanish is highly preferred.

 

HOURS PER WEEK: This is a full-time 40-hour week exempt salary position, utilizing a flex schedule.

 

TRAVEL REQUIREMENT: Minimal degree of travel to family homes.
